I am a Licensed Marriage and Family Therapist, LMFT
Certified Hypnotherapist, CH
I am a certified Prepare/Enrich facilitator and trainer. Prepare/Enrich is an assessment tool that is designed to highlight strength areas and areas of growth for couples in their relationship.

I am also a Certified Hypnotherapist and a National Guild of Hypnotists member. Hypnosis can help with phobias, fears, anxiety, smoking cessation, etc.

About Crystal
I am a Clinical Social Worker (associate) who values self-determination and seeks to affirm my clients' experiences while helping them to develop coping skills for dealing with life as it weaves its joys and challenges. I use a client-centered approach that is also trauma-informed and culturally sensitive. I believe we are all affected by trauma, and we are shaped by those who came before us. I enjoy cooking good food and sharing time with my family (as well as a good book or Netflix binge!).

We are all doing our best with the tools we currently have. If you are looking to add tools to your emotional toolbelt related to healing, self-compassion, mindfulness, communication, strengthening your relationships, or being the best version of yourself, I am ready for us to work together. We will set goals based on your needs and relationships and work toward those using various therapeutic approaches.

To my client:
I am pleased that you have chosen me to be your therapist. This disclosure statement is to give you information about my professional background, the services I provide, and the rights and responsibilities that will guide our work together. Please read this carefully and ask me any questions that you have.
My Education and Experience: I have an undergraduate degree in Family Studies and Cultural Diversity from Metropolitan State University in St. Paul, MN and a master's degree in Human Development from Saint Mary’s University of Minnesota in Minneapolis, MN. I am a graduate of Pfeiffer University in the Master of Arts Marriage and Family Therapy Program. I am a fully licensed therapist in the state of North Carolina.
My Services: I provide individual and family relational support services, including counseling and therapy. I work primarily with individuals, couples, and their families. I have training and experience in working with relational issues, depression, anxiety, sexual concerns, and trauma. I use a systemic approach, meaning that I will come from a “Big Picture” perspective regarding your relationships. My therapeutic approaches include, but are not limited to attachment theory, family systems theory, and emotionally focused couples' therapy. I am certified as a PREPARE/ENRICH facilitator, which is an assessment to provide services for couples. We will work together to develop goals for treatment.
Rights and Responsibilities: You have the right for me to keep your work with me confidential and I pledge to do this. I will identify you as my client only in the following situations: with your fully informed consent, if I become aware of your imminent intention to seriously harm yourself or someone else, if I become aware of on-going abuse or neglect of a child, elderly or disabled person, or if I were ordered by a judge to release information. Other than these exceptions, I will not reveal the fact that you are a client or anything about your treatment, diagnosis, or history.
Complaint Procedure: If you are dissatisfied with any aspect of your services, please inform me immediately.

Crystal's Professional Disclosure Statement
To my client:
I am excited about our work together. This disclosure statement is intended to provide you with
information about my background as well as the services I provide, and my responsibilities as a
therapist. Please read this carefully and ask me any questions that you have.

My Education and Experience: My undergraduate degree is in Social work, and I received it from
Walden University. My graduate degree is in clinical social work with a concentration in Trauma
Informed Care also from Walden University. I am currently working toward full licensure in North
Carolina under my clinical supervisor, M. Elizabeth Petree, LCSW, LCAS. I have worked with
diverse populations, concentrating on mental health, for nine years.

My Services: I provide individual, couples, and family therapy (as well as retreats and trainings)
using a culturally sensitive and trauma-informed lens. I have experience and training in working with
children and families who have intellectual disabilities, anxiety, depression, ADHD, complex trauma,
and issues with intimacy. I use various therapeutic approaches, including but not limited to Dialectic
Behavioral Therapy, Cognitive Behavioral Therapy, Compassion Focused Therapy, Bereavement
Therapy, Feminist Therapy, Narrative Therapy, and others. I use a Person In Environment and
Systems Theory approach coupled with these therapeutic modalities.
