Terms & Conditions

TherapyWorks, PLLC is proud to provide personalized support and care. The following agreement lists our billing, scheduling, and cancellation policies and procedures. If you have any questions, please reach out for clarification.

A.          Scheduling Services. All services can be scheduled by phone by calling TherapyWorks, PLLC at (336) 365-2354 or by emailing TherapyWorks, PLLC at admin@therapyworks-pllc.com. If the client schedules an appointment or communicates with the Provider via email, they are consenting for the Provider to respond to the email utilizing the same method, even if the client has not completed the email and text consent they receive in conjunction with this Agreement.

B.          Cost of Services. The Provider’s rate for a 50 minute visit/therapy session is $150.  

 

C.          Services. The client agrees to receive mental health counseling. The client understands the risks, benefits, and alternatives of receiving these Services and have had the opportunity to ask questions.

 

D.          Payment Methods. The client understands and agrees that payment for services shall be made at the time of service or using Alma’s billing platform. The Provider accepts payment in the form of cash or check or credit card. [If the client will be using insurance to cover some or all of the cost of any appointment, the client should call the Provider ahead of the appointment to ensure the insurance is accepted, and bring the insurance card to the appointment.] The client should be prepared to pay any co-payments at the time of the appointment with either cash or check or credit card. If the Provider is out of network for any insurance, the client must be prepared to pay in full for any appointment at the time of service, with either cash or check or credit card.

 

E.          Cancellation/No-Show Policy. It is understood that any appointment must be canceled at least twenty-four (24) hours in advance or the client will be responsible for a $75 payment for the missed visit.

 

F.           Confidentiality and Compliance. The Provider will take appropriate precautions to keep the client's health information confidential and to not disclose it without consent. The client is also protected under the provisions of the federal Health Insurance Portability and Accountability Act (HIPAA) and any other applicable federal and state laws related to protection of patient information, including but not limited to Public Health Law § 18. There are certain exceptions to when confidential information would not be protected—for instance, if the Provider believes that the client will harm themselves or another person or are neglecting or abusing a child or a vulnerable adult.

 

G.          Waiver of Liability. By agreeing to this document, the client agrees to waive, release, and discharge the Provider from any and all liability, including, without limitation, any injuries that may occur during the provision of services under this Agreement.
